AAA and Junior A tryout camps and conditioning camps require permission forms signed by Cochrane Minor Hockey.  Please obtain the appropriate forms from the Hockey Alberta website, complete the forms, and FAX or deliver the completed forms to the Cochrane Minor Hockey office.  The CMHA fax number is (403) 932-9347.  We will have the forms signed and forwarded to the club/team.  Note:  for AAA tryouts please use the Notification of Tryout Form.  For AAA conditioning camps you will need a separate Letter of Permission Form: Conditioning Camp.  AJHL teams have similar forms.
