2010/2011 Registration and Payment Information

ONLINE REGISTRATION is the preferred method for 2010/2011, 
BY MAIL - please print the registration package and mail all forms
Cheque(s) made payable to Cochrane Minor Hockey Association (CMHA)
Options: Full payment by July 15/10 or Two payments July 15/10 and Aug 15/10.  There will be a $30 service charge for all NSF cheques.

Online Registration will be automatically date stamped, however, your registration will only be considered complete once full payment has been received.
Mail - MUST BE POSTMARKED NO LATER THAN JULY 15, 2010.

Please note:  those selected to a AA team will pay the difference between the Rep rate and the AA rate AFTER team placement only.  This payment is due by October 1, 2010. 

Refund Policy
Full refunds will be given if you are not accepted due to space availability.  In all other cases a $50.00 administration fee will be retained in addition to any other prorated costs. Requests for refunds must be made in writing. The final date for any voluntary withdrawal with refund is November 15, 2010

Jersey’s – damaged or unreturned jersey’s will be charged $100.
