Reimbursement for Courses/Clinics

Please use this Coach Reimbursement form to be reimbursed for any Hockey Canada or Hockey Alberta required clinics and/or courses that you have completed and paid for to be able to coach for CMHA.

Once complete, please email the form and copies of receipts to: administrator@cochraneminorhockey.com

We will only accept and process reimbursements until January 15, 2024, so please email your form and receipts prior to this date.

Please allow a couple of weeks to have your reimbursement processed. Once your cheque has been prepared CMHA will email you that the cheque or EFT is ready for pick up or completed.
